A 58 years old diabetic, hypertensive housewife who eats too much presented to the hospital with sweating , sudden irregular palpitation & acute indigestion. She was on regular antihypertensive ttt & on regular hypoglycemic drugs. On examination of BP : 130 \ 105

Temp: 37.5 C.
Height:160 cm, Weight: 100 Kg.
ECG was done to her and revealed pathological Q wave & ST segment elevation in leads II, III, aVF .
